- Date of diagnosis / accident
- Time period booked off / Incapacitation period
- Name, phone number, and fax number of treating specialist
It is FMI's policy to settle claims within 48 hours, provided all documentation is submitted correctly and timeously. Please note:
STEP 1: NOTIFY FMI
Notify FMI of your claim by completing the online form, or contacting FMI’s Client Care team on 0860 10 11 19 / [email protected].
STEP 2: COMPLETE THE CLAIM FORM
A claim form, with a list of required supporting documentation (see below), will then be sent to the person who lodged the claim.

STEP 3: SUBMIT YOUR CLAIM
Once all the required supporting documentation has been received, and providing the premiums are paid up, a decision will made and the claimant will be notified.
